This is a very good question – something I don’t think we’re good enough at in the NHS. At the moment children who have care and support needs are provided for until adulthood and sometimes they can fall into the adult world where there is less support available, feel a bit lost and fall between the cracks. We should be better at preparing people for independence and I hope some of the changes happening in the NHS at the moment will help us to do this. One of the key challenges is supporting people with learning difficulties or disabilities to make healthy choices and understand the risks of making unwise decisions.
